(“Gotodoctor.ca”). This innovative program extends virtual care provided by physicians and pharmacists to include health coaching to drive sustainable behavior change for polychronic disease management.

Taking a patient-centered approach, GoToHealthCoach.ca provides personal health coaching to patients with anxiety, chronic pain, depression, diabetes, dyslipidemia, and hypertension, all delivered through a single platform. Eligible patients in Ontario are referred by their GoToDoctor physician or pharmacist to join the program and receive personalized support from a NexJ Health Coach between regular visits with members of their care team. A Health Coach encourages patients to increase their adherence to health positive behaviors, such as regular exercise, healthy diet, and mindfulness, as an evidence-based approach to help manage chronic conditions.

In the first 60 days since the launch of the project, the GoToHealthCoach.ca program has connected 92% of referred patients to a personal Health Coach to start making changes that will have a positive, lasting impact on their health.

Through NexJ Connected Wellness, patients use health trackers to identify trends in their health and wellness. Physicians and Health Coaches have access to the patient-generated health data and can gain insight into the self-management changes their patients are making. Patients can communicate via video chat and secure messaging with formal and informal caregivers, including their Health Coach, physician, pharmacist, and other health advocates. In addition, patients can read trusted educational content tailored to their condition(s), participate in daily exercise and mindfulness sessions, and connect with peers using community forums. NexJ Connected Wellness’ configurable gamification system is being used to incentivize patients by awarding them points for healthy behaviors including achieving tracker targets, completing workbook chapters, and contributing to forum topics. Points can be redeemed for rewards like gift cards and Fitbits.
